Lyra scopes describe the delegated capability under evaluation; they are not a blanket grant.

Decision	Meaning
allowed: true	The requested delegated authorization passed the current checks.
allowed: false	The policy, app, membership, scope, or step-up check denied it.
step_up_required	The requested scope is high risk and needs a recent passkey step-up.
production_action_executed: false	No downstream action was executed by this endpoint.
